Egg/embryo or sperm cryopreservation is available under the age of 44, limited to the enrolled employee or the employee's enrolled spouse (or domestic partner). Members must have a clinical evaluation and receive counseling by a reproductive endocrinologist regarding fertility and clinical candidacy for egg/embryo or sperm cryopreservation; decision making options involving egg/embryo or sperm cryopreservation as a fertility preservation technique; indications for egg/embryo or sperm cryopreservation; a description and understanding of, as well as the possible complications of, the medications and procedures involved in egg/embryo or sperm harvesting and cryopreservation. Members also must first exhaust any cryopreservation benefits that are provided under the health plan for fertility preservation when the member is otherwise eligible for fertility preservation as outlined in the Blue Cross and Blue Shield medical policy. Covered egg/embryo or sperm cryopreservation (including storage of cryopreserved eggs) is limited to a $20,000 lifetime benefit maximum for each eligible member. Note: The benefits available for elective egg/embryo or sperm cryopreservation are only available as of the date this rider is effective under this health plan coverage. This means any cryopreservation services that are received before the date this rider is effective under this health plan coverage will not be applied to the member's lifetime benefit maximum.
