Egg/embryo or sperm cryopreservation is available for members of any age who are not otherwise eligible for egg/embryo or sperm cryopreservation as described in the Blue Cross Blue Shield medical policy, limited to the enrolled employee or the employee's enrolled spouse (or domestic partner). Members must have a clinical evaluation and receive counseling by a reproductive endocrinologist regarding fertility and clinical candidacy for egg/embryo or sperm cryopreservation; decision making options involving egg/embryo or sperm cryopreservation as a fertility preservation technique; indications for egg/embryo or sperm cryopreservation; a description and understanding of, as well as the possible complications of, the medications and procedures involved in egg/embryo or sperm harvesting and cryopreservation. Covered egg/embryo or sperm cryopreservation (including storage of cryopreserved eggs) is limited to a $10,000 lifetime benefit maximum for each eligible member. Pre-service approval is required.
