Medex Choice covers one Medicare-approved routine pelvic exam, including a routine Pap smear test, every two years when the services are furnished by an HMO Blue provider, including a Nurse Midwife or Nurse Practitioner. This includes other Medicare-approved services furnished as part of an exam, such as a clinical breast exam to check for breast cancer. This coverage is provided once every year for members who are at high risk for developing cervical or vaginal cancer (as determined by Medicare). Refer to the Medicare handbook for a full description of Medicare-approved services. When Medicare does not provide benefits, Medex Choice covers one routine Pap smear test each calendar year when the test is furnished by an HMO Blue provider.
