Egg/embryo or sperm cryopreservation is available under the age of 44 who are not otherwise eligible for fertility preservation as described in the Blue Cross Blue Shield medical policy, limited to the enrolled employee or the employee's enrolled spouse (or domestic partner). Members must have a clinical evaluation and receive counseling by a reproductive endocrinologist regarding fertility and clinical candidacy for egg/embryo or sperm cryopreservation; decision making options involving egg/embryo or sperm cryopreservation as a fertility preservation technique; indications for egg/embryo or sperm cryopreservation; a description and understanding of, as well as the possible complications of, the medications and procedures involved in egg/embryo or sperm harvesting and cryopreservation. Covered egg/embryo or sperm cryopreservation (including storage of cryopreserved eggs) is limited to a $20,000 lifetime benefit maximum for each eligible member.
