Infertility technologies (such as in vitro fertilization and embryo placement, gamete intrafallopian transfer, zygote intrafallopian transfer, natural oocyte retrieval intravaginal fertilization and intracytoplasmic sperm injection). This health plan will provide benefits for covered IVF services for members over the age of 44. Members may be eligible for IVF without meeting the demonstration of infertility criteria for periods of natural conception and intrauterine insemination and may be approved for a fresh treatment cycle without having used all of their frozen embryos during a previous treatment cycle. Members (under the direction of their physician) may receive a multiple embryo transfer without having previously received a single embryo transfer. A yearly clomiphene citrate challenge test (CCCT) for members aged 40 thru 43 is not required prior to the approval of IVF. In addition, the basal FSH and estradiol requirement prior to the next fresh IVF cycle has been eliminated.
