The first visit for cardiac rehabilitation must be within 26 weeks of the date the member was first diagnosed with cardiovascular disease or after a cardiac event. Blue Cross Blue Shield must determine through medical documentation that the member has cardiovascular disease, angina pectoris or has had a myocardial infarction, angioplasty or cardiovascular surgery. (This type of surgery includes a heart transplant, coronary bypass graft surgery or valve repair or replacement.)
