Reciprocal in vitro fertilization (IVF) treatment for an enrolled female employee and/or the employee's enrolled same-sex partner. These benefits are available without first undergoing intrauterine insemination and without documented infertility. A pre-service approval is required from Blue Cross and Blue Shield in order to receive this treatment.
