Additional Weight Loss Program - you or one of your covered family members may be reimbursed up to $150 each calendar year for fees paid to health related activities approved by your employer. Qualifying activities may include smoking cessation programs, nutritional counseling (not covered by your health plan), and other weight loss programs approved by your employer group. You can request this reimbursement each calendar year; requests must be submitted to your employer group by March 31 of the following year. For information about how to become eligible for this reimbursement or for questions about approved health-related activities, you must contact your employer group. This Wellness Program is separate from your health plan.
