Infertility technologies (such as in vitro fertilization and embryo placement, gamete intrafallopian transfer, zygote intrafallopian transfer, natural oocyte retrieval intravaginal fertilization and intracytoplasmic sperm injection). Members may be eligible for IVF without meeting the demonstration of infertility criteria for periods of natural conception and intrauterine insemination and also, may be approved for a fresh treatment cycle without having used all of their frozen embryos during a previous treatment cycle.
