Fitness and Weight Loss Program - you and your covered family members can be reimbursed up to $200 per individual contract ($400 per family contract) for fees paid to qualified fitness/weight loss programs: in-person health club with cardiovascular and strength-training equipment; in-person fitness studio with instructor-led group classes; a virtual/online membership that provides cardiovascular and strength-training; home fitness equipment; or weight loss programs focused on healthy eating, activity, and include behavioral/lifestyle counseling with certified health professionals. Reimbursement is each calendar year and must be submitted by March 31 of the following year. You cannot be reimbursed for initiation fees, personal training, country clubs, athletic events/clubs; spas, instructional dance, pool-only facilities, gymnastics, martial arts, clothing, pre-packaged meals; and individual nutrition counseling. This Wellness Program is separate from your health plan.
