This health plan covers temporomandibular joint (TMJ) disorder treatment furnished by a covered health care provider up to the dollar amount described in the benefit limit. These covered services include:

$5 for each pair of standard or soft daily wear contact lenses.

$5 por cada par de lentes de contacto estándar o blandos de uso diario.

$5 for an initial supply of disposable or planned replacement contact lenses.

Note: A credit of $90 will be given for frames not in the collection.
