Telehealth services received from a Blue Cross and Blue Shield approved telehealth physician or practitioner using a telecommunications system. The telehealth provider will use an audio and video telecommunications system that permits a two-way, real-time communication. A member may see a telehealth provider online or by mobile device when care is needed for a minor illness or injury such as a cough, a sore throat, or a fever; or care is needed for a chronic condition; or care is needed for obstructive sleep apnea and other sleep disorders or sleep related conditions; or help is needed for a general health and wellness concern. A referral or prior approval is not required. For covered telehealth services, the member's cost share amount (such as deductible, copayment, and/or coinsurance) is the lowest cost share that applies for similar health care services when they are furnished in the office by the member's primary care provider. There is one exception, in-network member cost is waived for diagnosis and treatment of obstructive sleep apnea and other sleep disorders or sleep-related conditions.

This health plan includes the Blue Distinction Program. Under this program, covered services for the types of specialty care must be furnished in a Blue Distinction Center in order for the member to receive the maximum health plan coverage.

Transportation and Lodging: This health plan also provides coverage for transportation and lodging costs when the member receives specialty care covered under this program other than maternity care and the member lives more than 100 miles from the Blue Distinction Center. This coverage is available for the recipient (the member) and one companion (or two companions if the member recipient is an enrolled minor dependent child who is receiving specialty care).

Treatment to repair and/or replace sound, natural teeth damaged due to accidental injury. These benefits include accident care, treatment to repair or replace damaged teeth, necessary x-rays, and follow-up medical care. No benefits are provided for: dental prostheses (such as dentures, bridges, braces and other such dental appliances); and dental care following an injury caused by biting or chewing.

Fitness Program - you and your covered family members can be reimbursed up to $150 (in total) for membership and class fees paid to an in-person health club with a variety of cardiovascular and strength-training equipment; an in-person fitness studio that provides instructor-led group classes for cardiovascular and strength-training programs; or a virtual/online membership, subscription, program, or class that provides cardiovascular and strength-training using a digital platform. You can request this reimbursement each calendar year; requests must be submitted by March 31 of the following year. You cannot be reimbursed for initiation fees, or fees for personal training sessions, country clubs, social clubs (like ski, tennis, hiking), sports teams, spas, instructional dance, pool-only facilities, gymnastics, martial arts schools, and fitness equipment or clothing. This Wellness Program is separate from your health plan.
