Fitness Benefit - The subscriber or the enrolled spouse of the subscriber may take part of the fitness benefit. For each membership, up to a total of $200 per subscriber ($100 per enrolled spouse) for 50 visits in a six month period is reimbursed for fees paid for a health club membership or for fitness classes at a health club.
